    • Neuro-psychological evaluations
    • Speech pathology records or assessments
    • Occupational and physical therapy evaluations
    • Psychiatric evaluations
    • Legal documents including guardianship records
    • Educational Assessments including Individual Educational Plans (IEP)
    • Records from other providers
